Expert faculty present and discuss the advantages, disadvantages and pitfalls of approaches to pituitary and related lesions around the sella turcica. A point-counterpoint discussion emphasizes multiple aspects of surgical management, including both microscopic and endoscopic approaches as well as various methods of avoiding and dealing with surgical complications for these lesions.

After completing this educational activity, participants should be able to:

• Describe the relative advantages and disadvantages of the microscopic versus the endoscopic transsphenoidal approaches to sellar lesions.
• Identify complications and potential pitfalls with each of the different surgical techniques, along with their avoidance and management.
• Recognize the details of the surgical anatomy of the various lesions in the sellar region that can be approached using extended transsphenoidal operations.
